Do I need to book the lift for the cleaning as well?

In several managed blocks, yes. If the building requires a lift booking to move furniture, it often requires one for any contractor bringing equipment through, and arriving without it means waiting or being turned away. Ask the concierge when you book your removal and take both slots at once. Give us the time you are allocated and we will work to it.

Does the balcony count?

It does, and it is the item most commonly missed in a city centre check-out.
